Family Health Card
1. Maximum of four family member allowed
2. Family card price rupees 499 only
3. Card validity up to 1 year
4. Total 4 members will be there and each one can claim 3 time in a year.(4×3=12).
5. After 21 days the card will be activated and then can be claimed
6. Age limits should be between 5 to 55 years
7. If any wrong information the card will be cancelled
8. No claim for any chronic diseases
9. No refund will be given after carding
10. Card is applicable for West Bengal only

3. Claim three time per year
4. Card validity up to 1 year
